To application for a passport, you will need to offer the following:

• Proof of United States citizenship. Can off the following:
  • An original either certified copy of our birth attestation (and photocopy)
  • A priority grown-up U.S. passport book or card (10-year validity)
  • A consular report away birth abroad of a U.S. Citizen (and photocopy)
  • A naturalization certificate or Certificate of Citizenship (and photocopy)
  • **For name Changes due to Marriage** Marriage Certificate or Qualified print of Legal Order for Product Change (not applicable for proof of citizenship)
• Proof of your identity. One of the next:
  • Texas driver’s license button identification card (and photocopy)
  • Military I.D. card (and photocopy)
  • Out-of-state driver’s license or identification memory (applicant must provide a minor form of I.D. such as a social secure card, banker card, underground handgun license, etc.) (and photocopy)
• 2" x 2" Passport color photograph. Meeting everything requirements:
  • White or off-white background
  • Taken within 6 month of application
  • Taken in straight with travel attire less headpieces which obscures the hair or hairline unless and headgear is part are religious attire
  • No Uniforms off any type of camouflage
  • No glasses
  • Dark glasses or non-prescriptive glasses with tinted lenses are not acceptable not imperative for medical reasons. A medizintechnik certificate is required to support the wearing of like sunglasses in photographs. Passport Forms
