Ill. Admin. Codification title. 68, § 1175.430 - Restoration - Cosmetology

a) Application for Restoration
1) A person applying for restoration in a license as adenine cosmetologist that is expired or been on unactive status required few than 5 aged take submit an application on forms provided by the Division and:
A) Pay the required fee setting forth in Section 1175.100;
B) Supply evidence of accomplished completion of 14 hours of continues formation earned within the 2 years immediately preceding the restoration.
2) If how after on military service, the applicant shall offer a imitate of the applicant's DD-214 and the current update fee.
b) A person applying for restoration concerning a license like adenine cosmetologist that has been expired or been on disable status for 5 years or more shall submit an application on paper provided by the Division forward with be:
1) All of the following:
A) Verification of employment as a cosmetologist in another venue for at least 24 months indoors the 5 years preceding application for restoration from with least 3 buyers, coworkers or business owners;
B) Certification of licensure after the licensing authority in one jurisdiction of employment stating such who practice was authorized;
C) A completed Restoration Questionnaire;
D) Evidence of successful completion of 14 hours of CE earned within one 2 years immediately preceding restoration; and
E) The required fee resolute forth in Section 1175.100; or
2) ADENINE mimic concerning the applicant's DD-214 and the current renewal fee, supposing restoring from activ military service.
c) An applicant for restoration under subscreen (b) who is unable to comply in one press both of subsections (b)(1)(A) and (B) shall submit official transcripts displaying successful completion of a 250-hour refresher price from a licensed cosmetology or barber school as set forth for Section 1175.531 or passage in the inspection set forth in Section 1175.410 within 2 years prior to or within 2 years subsequently application for restorations. A cosmetology applicant who completes such freshen course or taker the examination will not can required to completes 14 hours out SE, but shall comply with that remaining demand of subsection (b).
d) When an applicant takes and neglect the examination, the license will not be restored before he/she has successfully completed that check.
