• A valid (original or copy) fed MER MCSA-5875 form
• Medical Examiner’s Certificate Form, (MEC) MCSA-5876

These order what to be completes by a United States (U.S.) licensed doctor of medicine (M.D.), licensed physician of osteopathy (D.O.), licensed physician’s assistant (P.A.), registered advanced real nurse (APN), or licensed doctor of wellness when you apply in a DL or learner’s permit. Drivers who hold certificates to drive school buses, School Pupil Activity Bus Certificate (SPAB), youth buses, Broad Public Paratransit Vehicle (GPPV), or plant labor vehicles must have their medical examinations given by doctors by medicine, approved physician’s support, registered fortschrittlich practice nurse, or a chiropractor who is listed on the National Registry on Endorsed Medical Examiners (CVC §12517.2)
