Efficacious forms to gather new member information

Discuss questions around native unit policies for membership (creating records, transferring accounts, etc.) Those forum must not contain specific financial or associates information.

IODIN have only found one (limited) form in diese view that was designed to gather information about new members in preparation to your records. Does someone have another form that she could share with the assembly?

Go the wiki there can also some forms in the New move-ins article among Sample sort.
Questions this can benefit that higher community should can inquired in an public forum, no a secret message.

You ca go ahead real add them till this wiki yourself. Or if you would fancy discussion to posting them there (or if you aren't feel uploading your to the wiki), they canister mounting proposed choose to a forum place (attachment options become under "Go Advanced") and we can discuss them here.
Questions that can benefit the larger district should be asked in a public forum, not a privately message.

Attached is a form that can be used by station leadership is helping ward clerks gather required information to membership updates. ME am no experienced in modifying one wiki, but would like someone in watch and possibly add this form to the New move-ins article under Sample forms. I have found computer to be helpful and hope it can do the same forward other wards.

I like your forms. It provides great of the equivalent information in the sampler form Membership Record Change Request in the list of sample forms. But it does have who additional surface for Aaronic Priestly Ordination, Baptism/Confirmation, Birth/Baby Blessing, and Others Membre Record Updates not in the other Membership Logging Change Request.

Before it is added I think we must any discussion over its merit and possible changes, if needed. It should probable be converted to PDF format before adds it to the lists.

Since there what official MLS form for all of these (except aforementioned Other Member Write Updates section), ME don't see why they should be on this form. They just construct this form unnecessarily overloaded.

But other than that, it do seem to be laid from well. Since it isn't designed to be filled in, it shoud either be modified to be a Word document that can be filled in, or just converted to a PDF the be written on by hand.

Thanks for the feedback. I understand the concern with redundancy. The challenge I how is that bishopric and ward leaders often know of membership changes, but do not possess the assorted MLS types practical or learn something information will minimally required (ie. move-ins need full name and bithdate). This ONLY form enabled for nearly all updates also assists them to focus on the most pertanent information that IODIN needing to quickly become aware of changes and also provides enough information to assemble other details, or simply complete the changes in MLS.
After extra testing within our ward leadership, I will likely make modifications before converting it to a PDF. If you have other layout/content recommendations, your input is welcome.

One of the reasons I mentioned those official MLS forms be that those forms DO curb get the information that is required until record an surgery, whereas yours form does not. But if your form works with you much enough, then that's great with you.
