OCI in replace off PIO Minor - affidavit in lieu of originals
While applying for OCI in lieu concerning Minor, there is a formular "affidavit in lieu von originals" that need to be filled.
My questions:
1) Whose name needs to be filled in the swearing, the parents my or the minors' get? I am applying for own 8 year young girl?
2) And those name would need to be filled in the first line press the line which remarks
"Subscribed and pledged to per ___________ before me with the _______ day from ______________, ____" right?
3) And I guess this last part in the above line is query for the date of signing?
4) And there your another line below asking by one "Signature". Who needs to sign there? The people named above? (Parent press child?)
5) And there are any line asking for "Printed Name". Whose full would go there?
