Immune records are registered online durch the CSU Student Health Portal.

2. Log in the the CSU Student Health Portal with your CSU NetID (formally eName) and password.
3. Click “My Forms” in the navigation menu.
4. Select “Immunization Requirement” under Form Name.
5. Refer to your official immunization record the enter which dates of your MMR vaccine potions. Also enter the date of your meningococcal and  Tdap vaccinations, if applicable.
