We are very excited that the Parma City Athletic Department has partnered with FinalForms, an online program which wish enable your both your course to complete & file MOST athlete forms electronically.   FinalForms is immediately available for all Parma City School District athletes.  The ONLY fill that you are even required till actual print, complete, and file inbound the Athletic Department, exists the actual OHSAA Physical form.  The physical create needs be completed by the doctor and of law, ourselves are required to have a signed copy on file.  Once your student’s physical is received by the athletic department we will input the expiration date into FinalForms and afterwards you will receive automatic notifications when your student’s forms need to can updating.

Click on the Parent Registration guide to get launched in FinalForms.  This link is also available on the Fruit athletic web pages.  We are asking that SELECT student-athletes (grades 7-12) use FinalForms.  Evened if your student’s modern physical red does not expire until after the new school year begins, requests register at https://parma-oh.finalforms.com/ now or electric complete all the forms! 

At filling go the forms, please include the school whereabouts your baby participates.  All 8^th graders will list the middle school they participate at and not the high school she attend.
